Personally I run the PRI my company has. Incoming is over the PRI, and I do
have a few VOIP-sourced DIDs that haven't given me any trouble, but
you are correct.
origination and termination are from the point of view of the PSTN. your origination provider allows PSTN subscribers to originate calls to you. Your termination provider allows PSTN subscribers to recieve calls from you...

If I remember correctly, nufone allows you to have a failover pstn number to
forward the DID to if your net goes down.
