Short answer: try new FXO modules or a new card. I've struggled with this for about a month. I've returned one TDM400 card, got a new one. Had same problems, Digium support installed a patch for zaptel, no difference. Then I diagnosed one FXO was dead. Got a replacement for that FXO. Suprisingly
