For PDFs, its impossible to read math directly. Your options there are to use InftyReader (which costs over $1,000) to OCR the math into LaTeX, or preferably, get the LaTeX source directly from the author.For word files, if you have MathType and MathPlayer

Math-ml should work in doc files or websites, not sure on pdf's though. Math-ml is the what the screenreader utilizes to read math equations.
