You're right about the router configuration. I'd only use NBAR if the wording
of the question led me in that direction.
Remember to consider QoS on the 3750 as well. Depending on how traffic is
marked, you may need to modify the CoS to DSCP mappings to ensure that whatever
hits the rout

Have you configured the hardware conference bridge on the gateway? Try a "show
sccp" on the gateway to see if your conference bridge is active.
Also, are you trying to establish the call between two sites? What codec is
being used?
