I've used it for reverse engineering a database, but haven't tried the other
way around (creating a DB from a ERD).
It worked pretty good... for what I was doing, open so
nstraints. The tool will look for that and use your constraints to
map the DB.

free with sql server 200 is Sql Enterprise Manager's diagram tool.

Have you tried simply making your relational diagrams with Enterprise
Manager. I don't know if you can get much better than that since it
comes with SQL server
