I would not try to tune that one server, it can maybe get a bit better, but
you will have troubles still.
How about setting up a box with just courier for the outbound mail?
You server could just relay everything to send away to this one and keep
his queue free. There you don't have to scan mail

I have all courier files installed under /usr/lib/courier. What folders
specifically should I move to a partition mounted with 'noatime'. I guess
/usr/lib/courier/var. Would it be best to do all of /usr/lib/courier.

That'd be the important one. The spool directories are best installed
in /var/spool (rpm install creates /var

filling my queue. At that point I had at least 2 messages in the queue
