The emacs backup file must have been the problem because I just have
this happen to me:
[EMAIL PROTECTED]:/etc/courier/smtpaccess# makesmtpaccess
Cannot store record for 127.0.0.1 - duplicate or out of disk space.
[EMAIL PROTECTED]:/etc/courier/smtpaccess# ls
default default~
[EMAIL
[EMAIL PROTECTED]:/etc/courier.old/smtpaccess# diff default~ default
[EMAIL PROTECTED]:/etc/courier.old/smtpaccess#
Ok, then the bug is no bug as this behaviour is described in the
documentation. However, it could be considered as undesired behaviour
that backup files aren't excluded. I

I was able to apt-get src courier-mta and run grep against it. I wasn't
sure which file exactly was responsible for having an extra 127.0.0.1.
My solution was to mv /etc/courier /etc/courier.orig, install everything
and then I simply copied the older smtpaccess/default file. I did not
modify

This fine November day, I ran an apt-get update; apt-get upgrade. The upgrade
failed.
