I just ran into this issue on Maverick. I could not reproduce it though.

I just experienced this bug with Lucid, and with Firefox specifically
experiencing issues with a full screen flash video. The message wouldn't
go away with xkill or itself, so usef "pkill gnome-panel" as someone
else here notes. Update Manager was run this morning, so I should be
fairly current.

Looking for a way to get rid of this dialog box I came across this bug
report. I am using Lucid. I do have Assistive Technologies enabled. I
am trying to come up with the steps to accurately reproduce this.

I've just experienced this bug as well. Ran pkill gnome-panel and the
problem was solved. After I initially tried to press escape, couldn't
do anything with that "Force quit" window - it just got in the way.

Still experiencing this problem using Ubuntu Jaunty Jackalope.
Still cant quite figure out exactly what causes it to do this. The
applet still works after the error (when the first window does not
disappear). So if you click on the "force quit" icon again another modal
window appears on top, but
I have had this bug a few times now. I get it if Firefox is having
problems and I try to kill it, but I get impatient and click the force
quit button over and over. pkill gnome-panel or rebooting makes it go
away.
Im running Intrepid Ibex,

I also just had this bug. I'm also running Firefox (like the original
report on both this bug and the duplicate) on the Intrepid LiveCD. I
solved the problem with pkill gnome-panel; when the panel reappeared,
the force-quit window had gone.

should change the status, so to be safe ill leave that alone.
Seems like this needs to be reopened as i now have this bug in hardy. Unsure
how it happened, will try and reproduce.

I can't reproduce it anymore too. Since I reported the bug I've done
some upgrades and apparently it's fixed.
