I have got the same problem on Ubuntu 14.04 64bit (with all available updates),
but I'm using the file manager Nemo (version: 1.8.4).
Closing all file manager windows and reopen them temporarily fixes the problem.
However this is very annoying if you have got a lot of windows/tabs open...

The sound worked fine until i plugged headphones in for the first time - then
it got stuck in headphones mode.

The difference with a text file is that it shows with a proper icon
rather than the same folder icon, and nautilus specifically asks to
overwrite rather than merge. But on symlinks nautilus asks Merge
folder? and then actually overwrites.

Yes - have a look at the network-manager-logind.patch that's attached to
this bug. That can be applied directly to /etc/init/network-
manager.conf, it just adds a pre-start section to the init script.

With my 14.04 LTS, evince no longer shows this issue, since my system has had
some extra GTK related libraries updates. Thank you for the help,
