at all reported bugs in a timely manner.
Further to your comments in #6 and due to the fact the the bug was not

RESOLVED: The problem is caused by a default selection which changed at some
point in Firefox releases.
In Firefox File==>Page Setup there is an option:
"Format For"
The default is "Any Printer for Portable Documents". If this is changed to a
specific printer, then the problem goes away.
Appare

Problem resolved. In "Page Setup", "Format for:" -- it must be set to the
proper printer rather than the default of
"Any Printer."

Some more information. I checked a machine in our University lab which has
Ubuntu 10.04 but Firefox
3.6.18 (it was last updated last September at the start of the school year).
Printing is correct with the headers/footers spaced from the top/bottom so that
they print.
I then upgraded to Firefox
Add package name (firefox 12.0+build1-0ubuntu0.10.04.1)
