Before you start tearing the rig apart, two ideas:-
-is there a big difference for weak signals of say S4?
-if not, it may just be the S meter calibration that needs attention.
-if so, preamp on for either? Att on for main?

The phones I use have separate mic and phones plugs - I'm only plugging
/ unplugging the phones. And the phones aren't the problem since it all
works fine in normal phone transmit. The problem (stopping the rig from
transmitting) only arises when playing a DVR message.

On VHF this effect can often be seen due to aircraft scatter, ie doppler
shifted returns as well as the direct one.
So lots of possible explanations.

Bit of a weird effect found here. I am using an external 2m transverter
to 28MHz. I noticed that when switching between VFOs using the A/B
switch from (eg) 145.7875FM to 144.430 usb, the KX3 says “Hi Sig” and
disconnects the preamp. This still happens with nothing connected to the
KX3's antenna
Since updating my K3 f/w to 5.01, I find that the time to switch between
VFO A and B has increased noticeably - it used to be near
instantaneous. This is not good when used in a contest and needing to
switch rapidly between frequencies. Anyone else noticed this?

I hear it as you do.
The amplitude of the transient depends on the received noise level. This
can be demonstrated with the rf gain control, but setting the gain
control low and then increasing the received noise level with eg a
preamp brings the transient back, so it's the actual noise
