Policy based email creation issue

With Exchange 2010 SP1 I'm finding if I have two users with the same name the user policy is adding a "2" or "3" to one of the email addresses even when the aliases are completely different. I'm sure it has something doFor example Smith, Jacqulyn (smithj) and Smith, Jacqulyn (smitj99). One is
