In IIS manager..the properties of the exchange virtual directories. Hit the
Directory Security tab, then edit 'authentication and access control'. Drop in
your domain for the default domain. I have that set on the exchange directory,
the exchweb directory and the owa directory. Not sure if you

Outlook Web Access tab. Right click owa (Default Web Site) select properties.
On the Authentication tab of the properties sheet under Use forms-based
authentication you can

I had that issue with one user also. Only way I fixed it was to recreate
his email account and start over. If you do this, don't forget to export
the mailbox to a pst before blowing away the mailbox :-)
