Hello Scott,
Is whatever you are plugging into the headphone jack *completely*
isolated from ground? The shield on the headphone jack is not at ground
potential and should not be connected to ground. Did you try plugging in
a stand-alone pair of headphones?

Just received my new 5000a and smiling ear to ear..no glitches setting up
and getting it running (PSDR V 2.016, Win 7 64 home premium).
BUT no sound out of the headphone jack except rhythmic, LOUD clicking. Mixer
was checked for headphones and powered speakers. I've tried two sets of
headphones.
