Thanks for all of the responses to my query. The mystery has finally been
solved: The tool used was AutoBookmark by Evermap. Apparently the person
created the bookmarks in FrameMaker, then used AutoBookmark on the PDF side to
delete the bookmarks, recreate them and create named

The Create Named Destinations for All Paragraphs does create named
destinations for every heading, but I'm not sure they're predictable
enough to use for anything. For example, for my Related Documentation
heading, instead of a simple name such as related_documentation, the
destination name is
Here's a discussion of a FrameScript that creates a TopicAlias marker
for every paragraph with a particular heading tag:

That could easily be adapted to create a named destination marker instead.
