FWIW, I have these paragraph formats available in all my documents. Including

OK, Autonumber Format works. Deja Vu Sans has the dingbat, if I set
the character tag to 24 pt and Subscript, it looks about right. I used
the Frame Above Pgf and Frame Below Pgf to add horizontal rules.

I use side-head paragraphs for this purpose all the time. Then I can enter
whatever I want in the side-head - whether it is a large icon like you asked
about, or other text (in red for example). With autonumber, you end up having
to use the same autonumber text.
