The same way you do the iPad and iPod.
Highlight a button and double tap and hold with two fingures and then
the label feature will come up.
Note that you have to be runing iOS five or later.

Hi Michael,
When I was flicking left and right with Voiceover on while holding the iPod
in portrait mode, I believe I only found two buttons. Also, how do you label
buttons?
