We use Cadence Allegro at work, and how they implement keepouts may be
interesting. There are several non-copper layers that are used to control the
keep outs and keep-in. In this tool the keep in / keep out is related to how
courtyards are defined.
Each library symbol (called a

design or not, obviously there are trade-offs, I am just trying to illustrate
the logic behind the recommendation so you can find what works best for you.

I've been reading this thread with great interest, as I have been bit by this
problem before. One thing I have seen done in the past in other pcb layout
software is to define the antenna component footprint as just 2 pins (one
signal, one ground) of the smallest size possible on the
(thats 2008-02-02).
