The form here http://www.gnhlug.org/forms/myth1.html is now live, in the sense that it accepts input, and mails it to maddog if all the checks pass. I tested it with Firefox 2.0, MSIE 7, Konqueror, and Lynx. The web pages are, I think, pretty well done at this point, modulo the remote control
