I interpreted the "can live without" as just an affirmation that
applications could continue to access the volume without failure while
the index delete/rebuild was in progress, not as a recommendation for
running that way long-term.

yet use NEWVTOC.
Extvtoc only works if the area next to the existing VTOC is free. Newvtoc will
re-locate the Vtoc.
Example follows:
REFORMAT UNIT(0E9F) NVTOC(ANY,90) VERIFY(HSM140)

You can dynamically expand an existing VTOC. Fixing a corrupted or
destroyed VTOC is much hairier.

I think your answer is to offload all datasets to tape, clip/init the pack with
a bigger VTOC and then reload everything back down. If it is SMS managed, then
you would QUINEW the pack, allow it to drain and then do the clip/init and put
it back in the storage group. Or make a new volume
