First of all, it looks like you did not update your cyrus.conf from a 2.0
version. That is the cause of the ctl_mboxlist vs ctl_cyrusdb error message
in the log.

You have to have c permission to delete the user. So, do a sam first, like
this:
localhost> sam user.fcuell cyrus all
where cyrus is your administrator. This gives user cyrus permission to do
anything he/she wants to the mailbox. Then,
localhost> dm user.fcuell
should work fine.
