I think I am making some progress. Now a message appears in the message_queue
table but it is has a status of failed and not email is received. Any ideas
what may cause this?

OK, so email is really enabled, and the table is empty. It sounds like
your settings are causing no emails to be generated.

Have you had a look in the mail queue table? It should have all the
generated emails, and have them flagged as to whether they've been sent
or not.

Thanks for replying to my question. Yes the AutoEmailOpacUser is set to send
and I have a valid email in the KohaAdminEmailAddress and the patron email
is valid as well. I don't see any indication of a mail message being sent
in mail.log or mail.err.
