Just coming to this late as I have been away for a while. I believe
the accepted way in the military (for example) is to use the
designation NFN (for No First Name) and, if applicable, NMI (for No
Middle Initial). Therefore, in your particular case, you'd probably
put [[NFN]] in the first

You can use either the Surname or the Given name field.
I would just use that name Nelson in the Surname field and leave the
Given name field blank. The key is how would you look this person up
if you had 10-20,000 people in your database -- I almost always use
the surname field to find

when you print reports and omit private notes, that info won't print
out, but you will still have it as a reminder in your database.
