Result (as you said) "The total number of individuals given in the
Surnames list excludes "Private","
I guess that makes sense in a way, and has nothing to do with AKAs or
married names etc.

Look for whom - Individual
Where to look - Private
How to look - Equal To
What to look for - Private

I do have a lot of "Private" Individuals, so that's probably the answer.
How would I test this? Can't figure out how to Search a Focus Group
for Private Individuals.

I tested this with the sample file and without tagging "include
invisible ..." I got 145 individuals (from 158) and after tagging this
option I had 158 individuals in the Surname index. The strange thing
is that there are no invisible individuals in the sample file (there
are 13 private individuals
