I think I would use the artifact template for the Vaccination card.
the template gives
Author last name, given name suffix (I would put the person whose name the
card belonged)
Agency/creator if applicable (US government or whatever agency? -- perhaps
World Health Org approved)

International Certificate of
Inoculation Vaccination Card; Source original copies

I think the template you are looking for regarding the letter is the
Letters Historic Privately held template
