When you say install a small boot server at each remote site, what is this
boot server's purpose, to be the DHCP server for the thin clients to allow
it to talk to a host site (or another site)? What if the router has a
built-in DHCP server?

I have a handful of Windows CD based thin clients that have a Realtek NIC in
them. You can hit Shift+F10 to get to the NIC's setup when the unit is
booting. Inside the NIC's bios there is an option for boorting using PXE.
You might try that.
