While I don’t think I’ve personally encountered this with the Item Chooser, I
have experienced it when using the Web Rotor, particularly when listing Static
Text elements. Of course I’m now unable to locate one of those problem pages to
post here for possible confirmation. :)

Use web rotor instead vo-u. I get something almost the same if I press
vo-command-n when in an open message in Mail. It simply crashes and
restarts VoiceOver!

more focused than the Item CHooser. Try it with cmd-u.
That being said, I’d report this one to Apple Accessibility. I don’t use the

What it is is a voice over crash. I've had this happen before but it's rare.
Can you maybe just find he text you are looking for by typing it in the box
when you hit vo f, then hitting vo g until you get to the text, or if the site
is well layer out you can navigate by heading with the vo cmd
When I am in Safari, not all but some times, if I want to use the item chooser,
I will press vo+i and after a short time of busy, the Mac will say, "welcome to
Mac OS 10; Voiceover is running." There is no item chooser list. I'm not sure
if it makes a difference how many items are on a page, but
