Does the file need to be a text file - I use pages and simply export the file
to word if I need to share with a word user either PC or Mac and it seems to
work fine and the formatting stays - I might have missed what it is you want to
do but thought I would share just in case it helps

Well, there’s always Emacspeak, for Emacs. It works with the GUI version of
Emacs even, so you just have to turn VoiceOver off and just use it. Or you can
use TDSR, a terminal screen reader for the Mac that is miles ahead of VoiceOver.

When I last looked, TextMate was the most accessible text editor for the Mac.
However, it hasn’t been updated for quite a while, and I’m wondering whether
there is anything else that should be considered nowadays.
