Sometimes when you hear security agent has no window, before
clicking anything, do the vo-shift-space. Sometimes you may be able to
find it with a vo-f2-f2 and see if you can find that window. This is not
a Mojave issue, its been around a while.

I've experienced something similar while in certain applications. What
happens is that it appears taht the Text Edit Box for some reason isn't
getting focus. I'm not sure if this helps you or not, but just figured
taht I would give it a spin as far as ideas are concerned.

I was trying to install some software.
What I ended up doing, is turn off VO, then cmd tab, then turn vo back on, then
cmd+tab back to the window, then vo+shift+space.
All that for that.

So I’ve updated to the newest version of the OS.
It seems, to me, that when the password window comes up, and i hit
VO+Shift+space, Vo just goes busy.
I have to force quit the app, maybe like 5/6 times, until I can get the
password to come up.
