2 - Yes, you can create and build a map dynamically.
The third one I don't know. I never have used Mapserver as tile server.

I am not able to reproduce with the msautotest tests:
These are the steps I have taken, in msautotest/renderers
- mapserv -nh
QUERY_STRING='map=point_ellipse.mapmode=mapmap_resolution=144map_size=800+600'
result/point_ellipse.png.png
- convert -scale 50% result/point_ellipse.png.png
No, they are not annotation layers.
I'll pull the latest svn and rebuild and if I still have issues, I'll
create a small test case.

msautotest/renderers/polygon-as-line.map
please submit a self-contained test-case where the issue can be reproduced.
