This subject has been reasonably well covered with the exception of those dudes that install so called "trick" wheels with a different offset and then the bearing load changes from the inner large bearing to the outer smaller bearing and dah the smaller bearing can not handle the load so bearing

Indications that bearings need to be replaced: 1. Mysterious vibration noise that gets louder every time you drive the car. 2. Wheel may make a noise if rotated on lift. 3. Wheel may make more noise when turning while under way. The longer you drive the car the more pronounced the indications

I would say you can likely tell when they should be replaced by the look and feel of the the bearing after you have washed it out with varsol or something similar. We just did this on a boat trailer that we acquired this spring. My younger son and I decided to clean and repack the bearings before
