Some fields can be disabled within SysConfig. See TicketPhone and/or
TicketEmail for those configuration (Agent and/or Customer). Any fields
Sysconfig doesn't address, you'll need to remove from the
Kernel/Output/Standard/(appropriatenamed).dtl
