I just couldn't get the new images to display so removed the lot and
re-installed which worked as you would have thought it would. Then I saw
bpa's re-work of the plugin and removed it again and installed that!

Originally I only did the cache.db as that was the only one mentioned. I
did rename the artwork.db
Factory reset didn't work so this must be being picked up from the
server somewhere.

I see in that post he is putting PCM volume up to 100%. I'm sure I have
read somewhere that 86% is the 0 gain level for PCM volume.

How do I fix the volume output to 100%? I'm using a Raspberry PI2 with
an IQaudio DAC+ to my amp. I know I can increase the volume in
Squeezelite but would like to set the volume to hardware rather than
software and to 100%.

Wiped card and re-imaged. IQaudio DAC available straight away after
