some time ago I had a similar problem in the UMN Mapserver. There I could solve
it, in which I have supplemented the corresponding epsg entries by a further
parameter: “+nadgrids=/usr/local/share/proj/”. This
caused the data to be transformed “on the fly”. Hope this helps.

At Kanton Solothurn we had the same requirement. And we did not want to fiddle
with the original proj.4 definition files. So we paid Sourcepole for solving
this directly in QGIS Server
(https://github.com/qgis/QGIS/commit/70863ecaf0ccfcb538e3892af4b528304b21a0a2).

Have you tried looking in the processing plugin? The ftools should be found
there since 2.16. If it's all messed up, rename the .qgis2 folder (hidden
file) and restart qgis.

The Open Layer Plugin is not the first choice for using basemaps anymore.
Much better and more stable ist the Plugin "QuickMapServices"

I am unable to run saga gis algorithms in qgis 2.16 from kyngchaos.com.
here is the first part of the error log. the list won't let me post the

I tried using saga_cmd from the commandline and I get the same error
(Segmentation Fault: 11)

"QuickMapServices" settings under "More services"

I had a chance to test on El Capitan - no problems. I used the GRASS sample
data from the GRASS website.
Can you try with the GRASS sample data? And try it in a separate GRASS DB.
