Sounds like either qmail-send can't make any sense of your queue, or
isn't running at all.

Thanks It looks like the queue was corupted I am waiting to see if the fix will
fix it or not.

outgoing remote queue. (As opposed to an average of a couple of hundred).

I was able to install qmail on my FreeBSD 2.2.2-RELEASE (ancient!) box. When I tried
to do the qmail-inject tests for LOCAL delivery, the messages essentially get stuck in
the queue, i.e. using
qmail-qstat.
For local deliveries, the maillog file shows the message log partially but I never see
Then get queue-fix from the qmail homepage.
It should sort out your woes.

We got rid of the /var/qmail/queue/info/[0-22]/*, mess/[0-22]/* and
