Some qmail rpm packages install a crontab entry which
over writes the /var/qmail/users/assign file with only entries
from /etc/passwd. I think it runs about once an hour

That's it. the qmail cron file in hourly.
Notice how it does things with assign files?
That's what is mucking everything up.
Remove this cron file and then every domain you
add after
