Found it
It seems that an RBL is not responding causing a 30 second timeout.
list.dsbl.org does not respond. Problem solved after removing it from
my RBL list.

Also the dns is internal and it shows no issues.
Also the dns is using a hosts file for all internal
systems so look up is only ~2 ms.

There are no entries in the tcp.smtp file. Can anyone suggest
where else I have to look for a delay?
