The logs look ok, but there's apparently no simscan happening.
You need to run
# qmailctl cdb
I did that ...still not sign of spam info in my headers
# qmailctl cdb

i had an issue where spam version did not update, maybe you can resolve your
issue with this.
stop qmail, then do a ps for any spamd/spamc/perl processes and kill them
rebuild spamassassin
rebuild cdb
start qmail.

I wouldn't go there yet, slamp. I'm not yet sure there is a problem.
Notice, the mail that's not being scanned is originating from his domain,
which is normal.

sorry for my great stupidity, eric you are right. everythink is working
fine. It was a new server. When I test from yahoo the headers works fine.
I worked for 2 weeks with this problem done 6 or 7 time fresh installs.

I am using Centos 4.4 and the latest stable package from qmailtoaster.
After installing everything simscan does not show in headers spammassasin
information, only clam is showed.
I have a problem with spamassassin.
When I run :
spamassassin --lint -D

To begin with, you need to run the command like this:
# sudo -u vpopmail -H spamassassin -D --lint
Is spamassassin running?
# qmailctl stat
What does simcontrol contain?
# cat /var/qmail/control/simcontrol
What does your SA control file contain?
# cat /etc/mail/spamassassin/local.cf
