The Cygolite Hotshot models come with a seatstay mount clamp in addition to
the seatpost clamp. I've found it can be mounted vertically and is not
particularly bulky, and the output can be lowered to be less blinding to
fellow riders.

We've built up tandems for road touring and brevets, all in Rivendell
style. Your list is good. Correct on the rear hub. The biggest cost drivers
on a tandem are cranks – we like Sugino XD-tandem specific with TA Zephyr
rings – and strong wheels.
We like White Industries hubs though they
1200K, Rambouillet, Boston-Montreal-Boston randonee 2004. Tough ride, 25mm
tires didn't do well on New England's pavement.
1200K, coupled Bleriot, Paris-Brest-Paris randonnee 2007. Terrific ride on
Grand Bois Cypress 32mm tires, even on chipseal.

like the noise is caused by

We use White hubs on our tandems. That rear clicking you describe is familiar.
It's the sound of a White hub needing service. The bearings are relatively
inexpensive.
