I use a Supraexpress 56i (internal K56flex) with flashable ROM.
It's plug and play but I jumpered the irq and base address. I'm
connecting at 46K most of the time. It works under linux,
FreeBSD, and NT and has been rock solid.

I've a Sidin 1456 56Kflex (Rockwell) external and it works just fine.
(but I don't know if Sidin is only an italian

I've got a motorola VoiceSURFR 56k.. It works excelently under 95
and NT and I've now got it running
