For some reason, it seems that sipx is not relaying the 'forward-to'

Another thing to look at is creating a user with NO PHONE, call forward that
internal user to the PSTN number, then call it. If that works, then try to
forward the call from the actual user to the "phanton" user and see if that
works.

It's most likely one of two things:
1. There is a problem with your rules and the call is not being forwarded
properly by sipx. A call trace from sipx would be very helpful.
2. The gateway is not handling the forwarding properly (i.e. I think the
grandstream has a strange way of handling "stage
