Joe, The AMA is supposed to put the forms on their web site. According to Steve Kaluf; You can call The Competition Department at the AMA, You can get the phone number from the website. Amy or Steve Kaluf, will be happy to send a package to you . If you have been to the Nats in the Last 2 years..
