I think you may have some leftover crap in your cache. Maybe due to
earlier crashes or uncontrolled shutdowns.
Try decreasing L1 to 10 and let Squid run for 24 hours without restart.

But I am still not convinced your problem is the cache growing beyond the
cache_dir size. If there was such problems in 2.5.STABLE1 I am pretty sure
we would have known about it already.

grow to 5 GB.
However, it's currently at over 8 GB and climbing.
