Can you try your tls cfg as below and make sure your certificates have the
right permissions.

method = TLSv1.2+
verify_certificate = no
require_certificate = no
private_key = /etc/letsencrypt/live/MYDOMAIN/privkey.pem
certificate = /etc/letsencypt/live/

checked and everything seems to be fine. Let me review my Kamailio config.
