Two things are needed to markup a Psalm title:
type="psalm"
canonical=“true”
When type has the value psalm, it can be styled differently and when it is
marked canonical it should always show.
Not sure how the result is handled by SWORD filters today. But SWORD should
allow styling of title with
OSIS has no problem with a footnote in a title.

A peculiarity may occur when the title is not [part of] verse 1, and when
the xref contains an origin marker.
Here's an example: (Welsh beibl.net)

Also, BPBible 0.5 (Windows) displays both AND distinguishes between them
using italics for the canonical title. AndBible shows both but does not
distinguish between them, placing the canonical title above the
non-canonical title.

Have discussed some aspects with Peter and with DM.
Which front-end app did you use to test this issue?
Have you tried more than one front-end?
Have you tried one based on JSword as well as one or more based on SWORD?
