I'm not sure about a tutorial however, once you click 'create
multipolygon', a new relation should appear in the section on the right (if
you can't see a box for relations you can select Windows at the top of the
screen and make sure that relations is checked). From there you can

draw the inner box,
select them both,
click on "Create multipolygon" ...
but that's where I hit trouble!

Does anybody know of a simple, straight-forward, how-to for creating

I did add a load of them yesterday, but was thinking it would be much
easier if they just created an account of their own & added them!
I'll hold off doing any more till something is decided.
Feel free to revert those that I've added if we decide they're not kosher.

This is unusual behaviour, certainly.
I can see that the notes in the US are being closed en masse as they
can't be verified through compatible sources.
On looking at the notes added in Australia, they do seem to be an
automated script comparing what's in OSM to an external source.
