I know of some grad programs that use exams at the end of two years to decide
whether or not you get nothing and have to leave (fail), a master's and have to
leave (a "Master's Pass"), or can go on to finish the PhD (a "PhD Pass").

There has been a lot of talk on the internet lately about perceptual learning.
I believe that most of it was spurred by an article on the topic that appeared
in the NYT. I have to admit that I don't know much about this topic, so two
questions for those of you more familiar with it than I:
1)
Have you been in grad school when the department has decided
not to allow a grad student to continue in the doctoral program but
decides to grant that student a so called "terminal" Masters degree?
