FWIW, installing initramfs-tools(-core) 0.132 from Debian buster/sid
fixed this for me. Both direct download and a build from source worked.

The problem still exists with the latest ubuntu kernel (3.13.0.34) but
then again you already know that as per bug #1326725. That bug report is
more than active than the one that I filed, so I suggest you use that
one, however I am still willing to allow you to double check using me if
you want.

and found the device does get created for the Playstation 3 gamepad and
it works as it should. I hope this kernel was the one you wanted me to
