I'm having this iproblem too now, but just since a few days ago. The
only thing that could have changed is I did some package updates. Here
are the latest ones:

... and killing pulseaudio processes seems to fix the issue if it is
there.

Ok, now audio is distorted again but now even mplayer is the same
distorted audio which was not the case before. It seems audio layer in
precise is totally unusable at least for me, cant mention more than some
hours when it was OK :(

Btw there was a hibernation process meanwhile, can it cause that audio
is totally bad for now? Oppps, now audio is OK again. Without doing
anything, no reboot, hibernation etc meanwhile since my last comment. I
don't know if it's realted but unity-2D shell is crashed report at the
very same time
And btw volume of audio is quite low even with max'ed volume. If I open
sound preferences I can boost the global volume level above 100% at the
output cfg, but after altering the volume later in the normal way, the
max is again too low, and I can start the configuration again :( Not so
handy. If
Now suddenly there is no sound in browser (not distorted, etc just no
sound) on my notebook without even a reboot. Again other applications
like mplayer works normally. Sound utilities shows the browser's audio
output in the audio client's list (of pulseaudio) but no hearable sound.

Hmm now audio works again for browser sorry, it seems starting mplayer
cures it?! I am really confused with this issue now ...
