I think i've found out how to REPRODUCE THIS 100% SYSTEMATICALLY, and
it's not random:
- open several windows of an application
- press CTRL+ALT+D so that you show desktop
- click on the application icon on the launcher once => will restore and bring
to front one of the app's windows
- click on t

This is tremendously annoying and it's only one of the endless
regressions in Unity. Canonical should SERIOUSLY consider rolling back
to Gnome which was much more stable and usable (or to whatever MATURE
and reliable, serious destop environment, unlike Unity which looks like
an alpha release). The
I have experienced this with several programs: Terminal, Nautilus,
Firefox for sure.
There is a workaround: alt+tab until you select the application of which some
windows don't show up, stop there but keep alt pressed. Then all the
application's windows are shown, and in this case it _does_ work
